The Team Summary Form should denote any team/venue issues and be accompanied with the appropriate team entry fee.
The Mixed Team Entry Form should contain all relevant player history.

The Bulk Registration Form should contain details of emergency players only.
The Change of Team Details Form should be used when altering the team's composition.
